Home Modification for Falls Prevention Program

The Home Modification for Falls Prevention Program provides free home safety assessments and low cost, high impact modifications to serve the goal of falls prevention. This is an innovative falls prevention program serving the High Country. The program is funded through an ACL Innovation Grant from the National Council on Aging (NCOA) and State Fiscal Recovery Funds, along with Occupational Therapy assessments provided by Appalachian State University's Occupational Therapy Program.

You may be eligible for this program if you:

  • are age 60 or older;
  • are concerned about your falling risk;
  • reside within Alleghany, Wilkes, Ashe, Watauga, Avery, Mitchell, or Yancey counties;
  • have not have participated in an evidenced-based falls prevention program, such as A Matter of Balance or Tai Chi, in the last 12 months;
  • and will not participate in these courses for the duration of participation in this program.

The Home Modification for Falls Prevention Program is currently accepting client referrals. Self-referrals are welcome.
